The Result of Primary Total Hip Replacement Arthroplasty Performed by Less-experienced Surgeons: Analysis of the patterns and factors of failure

Abstract
Purpose: To analyze the results of total hip replacement arthroplasties performed by less-experienced surgeons in order to evaluate the patterns and possible causes of failure.

Materials an Methods: Of 57 primary THRA (52 patients), performed between Jan. 1993 and Mar. 2000, 32 hips, in 28 patients, followed up for a minimum 2 years were included. The average duration of follow up was 52 months. The operations had been performed by six different surgeons.

Results: Postoperative dislocations were noticed in 12 (37.5%) cases, and radiological loosening was detected in 10(31.3%). Component migration was noticed in 2 acetabular and 7 femoral components. There were 6 cases of osteolysis. In 7 cemented stem cases, three showed cement failure or loosening. Re-revisions were performed in 7 hips. Twenty four hips (75.0%) experienced at least one complication.

Conclusions: The most frequent patterns of failure in total hip arthroplasties performed and cared by multiple surgeons in their learning curve, were dislocations and early loosening. To reduce these failure rates, quality control and standardization, through some forms of re-education, are mandatory.
